Pre-Camp COVID-19 Protocol Form and Instructions

Hello! Your Pre-Camp COVID-19 Protocol form will be available on Friday, October 14 at 6:00 a.m. Eastern. Read on for our Pre-Camp Protocol Instructions.

Our Pre-Camp COVID-19 Protocol Form is required of every attendee. This form contains COVID-related questions and our COVID test attestation. This form must be submitted between 6:00 a.m. and 9:00 a.m. Eastern time on Friday, October 14, 2022. If you do not complete this form, you will be turned away upon arrival—at Frost Valley YMCA or at the Shuttle Bus pickup, whichever you encounter first.

As a reminder, attendees experiencing any CDC-described symptoms of COVID-19 the week of Camp must submit proof of a negative PCR test through our COVID-19 Protocol Form. In our form, we ask you to confirm whether or not you’ve experienced COVID symptoms this week.

Please plan accordingly. To find a COVID-19 testing location near you, contact your local health organizations or your doctor. Check turnaround times for your local testing sites.

Regardless of symptoms, every attendee must provide proof of a negative COVID-19 rapid antigen test taken no earlier than 6:00 a.m. Eastern time on Friday, October 14, 2022. All antigen test results must be uploaded by 9:00 a.m. on Friday, October 14. FDA-approved at-home rapid antigen tests are OK. Please carefully follow manufacturer's instructions.

If you receive a positive PCR or rapid antigen test result, contact us immediately.
